Job Description:

Home Health Aide/CNA provides personal and hygiene care to our patients.
Home Health Aide/CNA follows the nursing care plan and reports to the primary nurse and/or the Home Health Aide Supervisor.
Home Health Aide/CNA is responsible for documenting daily visits and keeping a written report.
Home Health Aide assists patients with meal planning, preparation and feeding.
Home Health Aide may also perform light incidental household chores as necessary.

Requirements:

Successful completion of a nurse assistant or nurse’s aide training course approved by the State of Connecticut.
Connecticut State Nurses Aide Certification or Home Health Aide Certification required.
Responsible for completing yearly in services required by state law.
