JOB SUMMARY
The CNA is responsible for providing direct care activities for terminally ill patients. Activities can range from getting dressed, bathing, eating, and personal hygiene. CNA’s work closely with nursing staff. This position is “hands on”. Must support the Connecticut Hospices’ vision, mission, and operating principles.

ESSENTIAL DUTIES AND RESPONSIBILITIES
General:
- Provides direct care activities of daily living by assisting with serving meals, feeding patients as necessary and ambulating, turning, and positioning patients; and providing water and basic needs between meals.
- Answering patient’s call lights and any needed request.
- Reporting observations of the patient to assigned licensed nurse and or unit clinical coordinator.
- Provides adjunct care assisting assigned nurse with the administering enemas, non-sterile dressings, ice packs, heat packs.
- Checks vital signs and weights as needed.
- Room preparation for new admissions assigned.
- Changing soiled sheets, cleaning up spills, changing bedpans, setting up equipment, and reducing the spread of germs and infection in the patient care area.
- Assist with post-mortem care, releasing of decedents to funeral home, cleans patient area once decedents have been picked up from funeral home.
- Empties and records urine outputs.
- Completes Nurse to CNA Green sheets & q30 min. safety checks.
- Documents actions by completing forms, reports, logs, and records.
- Reports Inputs, Outputs, Meal percentages to assigned licensed nurse.
- Maintains work operations by following policies and procedures.
- Adheres to professional standards, hospital policies and procedures; federal, state, and local requirements; and Jcaho standards.
- Updates job knowledge by participating in educational opportunities i.e., Relias Education Portal.
- Adheres to The Connecticut Hospice “Mission Statement” and C.A.R.E.S values.
Health & Safety:
- Report and take required action regarding injuries, incidents, medical, and accidents required by policy.
- Make the health, safety, protection of rights, and the development of all co-workers and patients your primary responsibilities.
- Effectively manage medical and first aid emergencies.
- Follow all agency safety/medical related policies and procedures.
- Follow fire evacuation and disaster procedures correctly and in a timely manner.
- Report unsafe situations immediately; ensuring safety is imperative.
- Ensure pertinent emergency telephone numbers are readily available.
- Report symptoms and signs of communicable diseases immediately.
- Consistently use universal precautions.
- Report any incident that could potentially result in negative reactions from the community, law enforcement officers, and other agencies to the supervisor immediately.
EDUCATIONAL REQUIREMENTS, QUALIFICATIONS, and TRAININGS
- Must have a high school diploma, GED or equivalent.
- Must possess a State Certified Nursing Assistant Certification.
- Multi-tasking skills.
- Professional bedside manner.
- Basic knowledge of anatomy, physiology, and medical terminology.
- Ability to think and work independently and with direction and communicate with staff members in a fast-paced and sometimes stressful environment.
- Must have excellent communication skills and speak in public.
- Must pay attention to details.
- Capacity to maintain accurate records and exercise discretion in handling of confidential information.
- Willingness to work a flexible schedule; including at different sites and programs as needed.
- Ability to maintain assigned work hours.
COMPUTER AND SOFTWARE REQUIREMENTS
- Computer literate and proficient in Microsoft Office Word and Excel, Outlook, plus word processing and presentation software.
- Ability to operate media equipment such as tablets, smartphones, and other electronic equipment.
- Ability to work with general office equipment.
- Ability to work with and understand databases a must and the ability to learn technical skills.
PHYSICAL REQUIREMENTS & WORK ENVIRONMENT
- Shall have essential physical skills; be able to run, squat, stoop/bend, kneel, climb stairs, be able to lift at least 50 pounds (or ¼ own body weight), push/pull against potential resistance, and be able to implement non-physical and physical interventions and CPR with individuals supported when needed.
- Shall have adequate vision, reading, writing, and documentation skills, and hearing to perform the essential functions of the job.
- Must have enough endurance to perform tasks over long periods of time.
- Will be engaged in speaking, sitting, walking, driving, listening, and in communicating both orally and in writing while performing his or her duties.
- Must be able to listen and respond to questions and instructions.
